Once you have made an appointment with one of our specialists, you may complete the Client Information Form on line, if you would like. They are offered as a convenience to you to save time during the check-in process. If your appointment is with Dr. Thomas Evans, our Ophthalmologist, you should also complete the Ophthalmology Information Form, below. These forms can be either (1) e-mailed to the hospital or (2) printed and completed at home, then brought with you to the hospital appointment. Please submit these forms no less than 48 hours before your appointment if emailing.
